Visiting CMS requires a prescription from a referring physician.  Discuss the CMS option with your physician and inquire about a prescription to see us for concussion rehabilitation.

New Patient Information
Please download and review the new patient information and forms found below.  Complete all forms and bring them with you to your first scheduled appointment.  Please note that not all services provided by CMS, PLLC are covered by your insurance.  All of our fees will be explained upfront during your initial visit, and no tests or services will be performed without first explaining any fees that may be incurred.
